Service agreement template
A service agreement defines recurring or on-call work — scope, response times, rates, term, renewal, and cancellation — so maintenance routes and small-job customers know what they bought before the first visit.

Agreements vs one-off estimates
Maintenance, monitoring, and priority service plans need recurring terms — auto-renew, price adjustment, and cancellation notice — that a single estimate does not cover.
Use this document for the relationship frame; attach a price sheet or Schedule A for visit definitions and rates.
Auto-renew and price changes
State auto-renew laws may require specific disclosure and cancellation methods. Say how you notify price increases and how either party terminates.
Define what is excluded — storm cleanup, parts, after-hours multipliers — so the first emergency call is not a surprise bill.
Required clauses
Recurring revenue without written renewal and cancellation rules becomes churn and chargebacks.
- Parties and service address(es)
- Description of services and visit frequency
- Fees, billing cycle, and payment method
- Term, renewal, and cancellation notice
- Response / SLA (business hours vs emergency)
- Customer access and safety responsibilities
- Insurance and license statement
- Limitation of liability and dispute terms (per counsel)

Visit definition
“Each ‘visit’ includes up to [hours] on site for listed tasks in Exhibit A. Additional labor billed at $[rate]/hr with customer approval.”
Prevents ‘you were only here twenty minutes’ disputes.
Price adjustment
“Fees may adjust annually with [days] written notice, not to exceed [percent] per year without customer’s right to cancel.”
Protects margin when fuel and wages move.
Common mistakes
Vague visit lists and silent auto-renew are the main failure modes on service plans.
- Unlimited ‘priority service’ without defined response time
- No exclusion for parts and materials on maintenance
- Missing cancellation notice period
- Verbal renewals without updated signature or email trail
